Our Policies

APPOINTMENTS:  Office visits are by appointment.  Please call to make an appointment.  Sick visits will be scheduled within 24 hours.  Walk-ins must wait until patients with appointments have been seen.

  • Please bring your insurance card, co-payment, and all medications with you to each visit.

  • Cancellations:  Please give 24-hour notice for cancellations.  This will make appointments available for sick visits for all our patients.

  • Emergencies:  Please call the office first, if possible.  Whenever the office is closed, the answering service will contact a physician on call for you.

TEST RESULTS:  We will contact you with all test results (lab work, x-rays, etc.) as soon as we receive them.  If you have not heard from us within two (2) weeks after your test, please call our office.  You may also view some test results by accessing your on-line Patient Portal.

 

PAYMENTS:  Payment is due at the time of visit.  If you are insured, your co-payment is due at the time of visit.  Professional services are rendered and charged to the patient and NOT to the insurance company.  You, the patient, are directly responsible for payments to the doctor.  A health insurance policy is a contract between you, the subscriber, and your insurance  company.  As a courtesy, we will submit claims to your health insurance company.  If no response is received within 45 days, you will be directly responsible for any balance due.  WE DO NOT ACCEPT MOTOR VEHICLE OR WORKERS COMPENSATION INSURANCE.  You are responsible for all balances for treatment at this office.

 

Release of Information and Assignment of Benefits:  We will ask you to sign a document authorizing release of medical information to your insurance company and assign payment of benefits to Jack Chang, M.D., P.C., for all claims processed in connection with your medical treatment.  You are responsible for any non-covered services.

 

Medicare Patients will also be required to sign a document accepting responsibility for all non-covered services.

 

INSURANCE:  We accept most major health insurance plans, including the following:

  • Blue Cross Blue Shield

  • Fallon Direct & Select Care

  • Harvard Pilgrim

  • United Health Care

  • Medicare, along with most secondary insurances

  • Tufts Health Plan, including Tufts Medicare Advantage HMO/PPO plans.

  • We do accept other Medicare Advantage Plans that are PPO ONLY. 

If you have an HMO plan, please be sure Dr. Jack Chang is listed as your primary care physician.
 

REFERRALS:  Please be advised that we coordinate your health care with physicians within our local network. Please allow at least 7 days for referrals to be processed.  Please contact our office BEFORE making an appointment.


PRESCRIPTION REFILLS:  Please call at least 48 hours before your prescription runs out. We request patients call for all refills, not the pharmacy. You may call pharmacy to check if prescription is ready to pick up.  Controlled-substances, as well as some narcotic prescriptions, cannot be filled without an appointment.

BILLING:  Please call the office if you have any questions about billing. We accept all major credit cards, as well as cash or check for payment of balances due.
 

NO-SHOW POLICY:  Please call the office 24 hours in advance if you need to cancel or reschedule your appointment.  We do charge $25.00 if you miss your appointment.  Thank you for your cooperation.


LANGUAGES:  English, Chinese & Spanish are spoken at this office. Please be sure to bring a translator to each visit if you speak any language other than above.
